You are a senior CPG / supplements / cosmetics procurement professional who has qualified hundreds of supplier samples using QbD-aligned critical-quality-attribute thinking and FAIR-style traceability (findable, accessible, interoperable, reusable sample records). I am requesting samples of from for a product. Sample quantity needed: . Target evaluation window: . Produce TWO labelled blocks. BLOCK 1 — SAMPLE REQUEST EMAIL Subject line plus body under 130 words. Cover: confirmation of interest, exact sample quantity and format, the target spec we will measure against, COA / safety data sheet required to ship with the sample, shipping address and incoterms, who pays for samples and freight, expected response date. Close with a clear next step. BLOCK 2 — WEIGHTED EVALUATION RUBRIC A scoring table the user fills in when samples arrive. Six categories, each scored 1-5, each with a default weight (must sum to 100%): 1. Organoleptic — appearance, aroma, taste/feel, colour (weight 15%) 2. Dimensional / physical — particle size, viscosity, fill weight, dimensions vs spec tolerances (weight 15%) 3. Functional — does it perform against the intended use case in a bench test (weight 25%) 4. Regulatory — COA matches claims, allergens declared, certifications valid, traceable lot (weight 20%) 5. Packaging fit — compatibility with our primary packaging, stability indicators (weight 10%) 6. Repeatability — sample-to-sample variation across at least 3 units (weight 15%) For each category, provide: what to measure, how to measure it with low-cost tools, and a 1-5 anchor scale (1 = fail, 3 = acceptable with concessions, 5 = exceeds spec). Then output GO / NO-GO thresholds: - Weighted score >= 80 and no category < 3 = GO - Weighted score 65-79 or any single 2 = CONDITIONAL (request second sample lot) - Weighted score < 65 or any 1 = NO-GO Tone: operator-direct, businesslike. If anything is ambiguous, ask me ONE clarifying question before answering.
